Job Description
Mission:
Reporting to the Director of Care, the RAI-MDS Coordinator is responsible for supporting the longterm care home in the management and sustainability of RAI-MDS processes to ensure the validityÂ
of clinical and quality outcomes that support the provision of care and services, in accordance withÂ
best practice, interRAI and legislative requirements.

Key Responsibilities:
â–Ş Coordinate and monitor RAI-MDS processes within the home according to organizationalÂ
policies and procedures, legislative requirements, standards of practice and inter-RAIÂ
requirements (CIHI)Â
â–Ş Develop and maintain a current Resident assessment schedule and ensure it is accessibleÂ
to all RAI users.Â
â–Ş Maintain efficient processes for data collection to ensure a complete and accurateÂ
assessment that supports an individualized and comprehensive Plan of Care.Â
â–Ş Facilitate RAI-MDS and application training (Point Click Care, Point of Care), provideÂ
ongoing leadership and mentoring assisting with knowledge application to support dataÂ
accuracy and proficiency of all users.Â
â–Ş Implement processes that support identifying gaps/discrepancies and provide ongoingÂ
education to reinforce knowledge, promote capacity and enhance staff competency.Â
â–Ş Monitor RAI-MDS assessments, resident assessment protocols/clinical assessmentÂ
protocols and care plan reviews for completion in accordance with required timelines.Â
â–Ş Monitor, analyze and evaluate generated outcomes to support continuous qualityÂ
improvement.Â
â–Ş Generate RAI-MDS output reports and in collaboration with the interdisciplinary team,Â
facilitate their use in clinical and operational decision-making.Â
â–Ş Maintain ongoing review/audits of MDS assessment data elements and all resourceÂ
utilization groups for accuracy and optimization potential prior to quarterly submissions.
â–Ş Manages RAI-MDS submissions within required timelines, review CIHI reports and MDSÂ
corrections and resubmissions when necessary.Â
â–Ş Manage and support all Relias LMS users in the home and ensure successful completion ofÂ
the annual competency evaluations.Â
â–Ş Perform other duties as assigned.Â

Required Education, Credentials and Experience:Â
â–Ş Currently regulated or licensed health care practitioner.Â
â–Ş A minimum of three (3) years clinical, project leadership or management experienceÂ
â–Ş Demonstrated interest in the RAI Coordinator role and MDS program.Â
â–Ş Minimal 1-year experience in the RAI Coordinator role (preferred)Â
â–Ş Current knowledge of RAI-MDS 2.0 and experience in the use of Point Click Care systemÂ
and Point of Care is an asset.Â
â–Ş Thorough knowledge of LTC legislation, program standards, including documentationÂ
â–Ş Demonstrated success leading new initiatives.Â
â–Ş Effective inter-departmental communication and leadership skills are necessary.Â
â–Ş Demonstration of well-developed oral and written communication skills.Â
â–Ş Knowledge and experience with adult education strategies and facilitation.Â
â–Ş Previous experience presenting to groups.Â
â–Ş Well-organized, with excellent time management skills.Â
â–Ş The ability to work with and relate to interdisciplinary team members and other internalÂ
stakeholders.Â
â–Ş Strong and effective problem-solving abilities.Â
â–Ş Proficient computer skillsÂ
â–Ş Current (within 6 months) Vulnerable Position/Sector Screening (VPS).
